POSITION SUMMARY:

Reporting to the Director of Emergency Housing, the Registered Nurse case Manager will provide case management in the Long Beach group home. RN Case Manager will provide coordination of medical, nursing and social services for residents. In addition, will provide education to staff and implement service plans.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Assure coordinated, quality provision of AIDS services for each Alliance for Housing and Healing/Serra Project resident through close supervision and coordination of medical, nursing, and social services; ongoing education of staff and residents; development and maintenance of comprehensive service plans, and referrals to appropriate AIDS related services as needed. Follow all Alliance for Housing and Healing/Serra Project policies and procedures and follow all contracts with our government agencies, Title 22, OAPP, Community Care Licensing, HOPWA Perform comprehensive assessment of the client’s physical, psychosocial, and functional status within two weeks of the resident’s admission to Alliance for Housing and Healing/Serra Project. Develop a Comprehensive Individual Service Plan in collaboration with a team who may consist of the resident, family, staff, physician, nurses, pharmacist, social worker, and others as necessary. This service plan is communicated to staff in order that they may assist in implementation. The Individual Service Plan is reviewed and updated every three months and as necessary. Meet with the care team/management for resident monthly case conferences. Communicate with AIDS service providers on an ongoing basis to assure coordinated, quality care. Make referrals to community resources as appropriate. Document all contacts made with Individual Services Team members. Available to staff for consultation regarding resident Individual Service Plan or specific problems. Coordinate and monitor the documentation and safe administration of all resident’s medications as specified in the policy and procedure manual. This includes monitoring the administration of controlled substances and providing our residents with injections as prescribed by their doctors. Coordinate resident discharges from the hospital back into Alliance for Housing and Healing/Serra Project. Assure access to services, advocate for client needs, monitor service delivery, and evaluate service outcomes. Make weekly visits group homes to meet with residents as needed. Maintain compliance with state mandated regulations for Residential Care Facilities for the Chronically Ill.
Must participate in company audits and assist with submission of essential documents to maintain compliance.
Provide education to staff as needed regarding residents health problems, medications, and special circumstances.
Development and updating of the residents Individual Services Plan, Implements, and reviews healthcare plans for residents that are living with chronic illnesses
OTHER DUTIES MAY BE ASSIGNED TO MEET BUSINESS NEEDS.

REQUIREMENTS:

Training and Experience: Must possess an active RN license. Minimum of two years of experience in community. 1 year community nursing & 2 years clinical nursing will be accepted in lieu of 2 years community nursing. Experience in providing direct care HIV/AIDS individuals required. Provide an active CPR and First Aid card.

Knowledge of:

Clinical nursing assessments, case management practices, health care systems, AIDS and HIV-related illnesses including common comorbidities. Microsoft office.

Knowledge of clinical and social services documentation procedures and standards.

Ability to:

Assess biopsychosocial needs, utilize community resources, and communicate effectively. Respect the dignity of clients who may have experienced stigma or discrimination; prepare accurate, concise, comprehensive reports; maintain confidentiality of the clients being serviced, maintain client records in an accurate and timely manner. Demonstrate proficiency with written and verbal communication skills. Meet reasonable deadlines.

Provide Alliance for Housing and Healing/Serra Project with yearly Physical and TB tests and copies of all in-services trainings.

WORKING CONDITIONS/PHYSICAL REQUIREMENTS:

This is primarily an office position that requires only occasional bending, reaching, stooping, lifting, and moving of office materials weighing 25 pounds or less. The position requires daily use of a personal computer and requires entering, viewing, and revising text and graphics on the computer terminal and on paper.

SPECIAL REQUIREMENTS:

Must possess a valid California drivers license; proof of auto liability insurance; and have the use of a personal vehicle for work related purposes.
