We are currently seeking a Director of Population Health Management in our Manhattan location

Position Summary: The Director of Population Health Management (DPHM) is responsible for developing, implementing and evaluating Quality Improvement /Quality Assurance (QI/QA) and Risk Management program and training activities, including principles of QI/QA and methodology used by Apicha CHC.  In collaboration with, and under the direction of, the Director of Medicine, the DPHM assists in defining opportunities for Quality Improvement and Risk Management, and collaborates with multidisciplinary staff to improve patient experience, satisfaction, and sustainable outcomes.  The DPHM understands and maintains awareness of the changing healthcare landscape, including evidence-based quality improvement, reimbursement reform, and delivery system transformation.

 

Qualifications: Bachelor’s degree with experience working in healthcare setting required; An advanced degree in related fields including MPH, Nursing, MHCA, is strongly preferred; Experience in supervising paid staff and project management strongly preferred; Familiarity with Quality Measures such as HEDIS, UDS preferred; Experience with NCQA accreditation standards preferred; Strong oral and written communication skills required; ability to interact within all levels of the organization; Ability to work in A rapid paced environment with team of diverse background required; Strong knowledge of Microsoft Office products required; Experience with Analytic software required; Demonstrated ability to successfully plan, organize and manage projects;  Detailed oriented, excellent proofreading and editing skills; experience working with medical software;  eClinicalWorks strongly preferred
