JeffCare, a program of Jefferson Parish Human Services Authority (JPHSA), is a Federally Qualified Health Center (FQHC) comprised of two Health Center sites. JeffCare’s goals are an integration of primary care and behavioral health services to best serve the needs of the population; and, the empowerment of individuals to achieve an improved quality of life through the provision of effective services, delivered in an environment of integrity and respect.

The position requires an individual highly organized and skilled in the coordination and monitoring of the delivery of primary care, behavioral health, and integrated services. The RN Clinical Quality Program Monitor requires a person with strong attention to detail, excellent communication skills, coordinate with other professionals, and the ability to multi-task. This individual must also be highly knowledgeable in the best practices in the delivery of primary and behavioral health care as well as skilled in the coordination of care across service delivery areas.  The incumbent is responsible for the oversight and monitoring of the provision of quality care within JeffCare, a program of JPHSA, as well as participating in the coordination of care across JPHSA services and external referral resources.

This position requires a registered nurse with current licensure to provide oversight and monitoring of the quality of care and coordination of care to individuals served within JeffCare and JPHSA as follows:

85%  Performance planning and monitoring of the quality of care provided by JeffCare

  • Analyzes the processes and procedures used in patient care and ensures all individuals served receive care based on these standards.
  • Monitors data required for performance indicators and other reporting requirements to ensure it is collected appropriately as well as to ensure performance indicators are met.
  • Monitors coordination of care provided by Primary Care providers to ensure Patient-Centered Medical Home (PCMH) Recognition standards are consistently met including, but not limited to: standards specific to the systematic tracking of tests results and coordination of care across specialty care; facility-based care; and, community organizations.
  • Monitors Healthcare Effectiveness Data and Information Set (HEDIS) measures, and provides feedback to staff regarding required documentation and/or services to improve JeffCare’s measures.
  • Monitors data required for performance indicators, Uniform Data System (UDS) performance measures, and other reporting requirements to ensure data is collected appropriately as well as to ensure performance expectations are met.
  • Reports on staff compliance with JeffCare treatment protocols to JeffCare Leadership, and addresses how compliance or lack of compliance impacts the effectiveness of each provider.
  • Provides recommendations and implementation support when processes and procedures do not support improved patient health.
  • Chairs the Performance Quality Improvement Committee.
  • Works with the Performance Quality Improvement (PQI) committee to propose and implement corrective action plans when issues are identified.
  • Manages data gathering, compilation, and analysis of JeffCare customer satisfaction surveys.
  • Presents reports to the JeffCare Board of Directors as requested by the JeffCare Divison Director.
  • Promotes ongoing support of Council on Accreditation (COA) and PCMH certification by assisting with procedure development and data collection.
  • Provides education to JeffCare related to performance improvement, clinical patient safety, and statistical analysis.
  • Participates in risk management through the identification, trending, and follow-up of clinical issues.

10%  Supervision and Training

  • Supervises the Quality and Assurance Coordinator and Registered Nurses and Nurse Population Health Manager.
  • Performs supervision per the JPHSA Staff Development and Supervision Guidelines.
  • Completes PES plans and ratings for direct reports, as required.
  • Participates in the development of corrective action plans and training as issues are identified.

5% Completes other duties as assigned to meet the needs of the Health Center

  • Complete all required in-service/ trainings in order to keep skills current as well as adhere to all JPHSA and JeffCare policies and procedures, accreditation standards and Civil Service Rules.
