Job Description
The major responsibilities of the Program Coordinator Sr. Connect for Youth is to lead and implement Lurie’s SAMHSA Navigator 5-year project (Connect for Youth-C4Y).  C4Y is a collaboration with Haymarket to implement a citywide, trauma-informed, integrated substance misuse and HIV prevention strategy for youth of color ages 16–25 in Chicago.

 

The C4Y Sr. Program Coordinator will work closely with Lurie’s Adolescent Medicine clinical and community program team as well as Haymarket to 1) Increase understanding of the intersection of HIV and substance misuse and the importance of prevention/treatment among youth; 2) Increase access to comprehensive, youth-friendly integrated screening and testing for HIV, viral Hepatitis, and substance use; 3) Increase linkage to, engagement in, and retention in care for low-income and high-risk youth of color; and 4) Increase capacity of other adolescent-health providers to address the intersection of trauma, youth substance use, and HIV to raise awareness of the HIV Care Continuum with youth of color.

 

The Sr. Program Coordinator is responsible for the day-to-day implementation of the project, coordination with Lurie and Haymarket staff, meeting deliverables, and regularly reporting outcomes.

 

Essential Job Functions:

Collaborate with Lurie Children’s Substance Use Prevention Coalition (SUPC), Youth Advisory Board, city agencies, and other community partners to identify and implement screening and educational opportunities
Promote testing and educational events via flyers in targeted neighborhoods, partner engagement, and
online outreach through social media, particularly apps
Receive training on and oversee the implementation of interventions (motivational interviewing, trauma informed care, and substance use screener)
Support the development and implementation of HIV/substance use prevention education and public messaging campaign
Collaborate with evaluations team to create and implement project evaluation
Report annual outcome data reported to partners & stakeholders
Deliver HIV/hepatitis and substance use screening/testing
Oversee project and ensure deliverables are met
Coordinate with Haymarket to integrate Linkage to Care Navigator into the project and workflow
Maintain necessary databases and submit all reports including GPRA and Continual Quality Improvement
Manages and maintains the project budget
Monitors and orders testing/screening and education materials as needed.
Enthusiasm for working in community spaces and ability to work evenings and weekends when needed
Participates in Hospital, Division, and community meetings/events
Performs job functions adhering to service principles with customer service focus of innovation, service excellence and teamwork to provide the highest quality care and service to our patients, families, co-workers, and others.
Other job functions as assigned.
Knowledge, Skills & Abilities:

Master’s degree in a related field or 5+ years of relevant experience; health-related background highly desired, with HIV and/or substance use experience preferred.
Five or more years’ experience with project leadership; direct community experience is essential, experience in Illinois and Chicago is preferred.
Demonstrated ability to manage a project, grant deliverables and partnerships with outside agencies
Commitment to trauma-informed care, evidence-based and community-driven processes.
Experience with linkage to care services and community education
Strong analytical, interpersonal, managerial, and oral and written communication skills required.
Strong working knowledge and skills in Microsoft computer software applications and database technologies.
Experience working with youth, particularly racial and ethnic minority young people
Ability to take initiative and work independently
Demonstrate fluency and clarity in written and oral communication. Demonstrate knowledge of programmatic scopes, including the ability to present the project objectives fluently.
Ability to establish priorities and react promptly to a large variety of requests and needs, sometimes in stressful situations.
Demonstrated ability to function as part of a multi-disciplinary team.  Strong organizational and communication skills.
Valid State of Illinois driver’s license with access to reliable transportation in order to attend and/or facilitate off-site educational programs preferred.
Fluency in Spanish preferred.
Ability to establish priorities and react promptly to a large variety of requests and needs on short notice.
