Overview
Public Health & Wellness Program Lead
We are seeking a Public Health professional to be the lead in implementing an exciting new Public Health and Wellness Program on the reservation of a federally recognized Native American Tribe located in the Olympia, Washington area. This would be a full-time position(On-Site) and would entail responsibility for extending care and wellness efforts beyond the clinic walls.
Representative Job Duties will include:
-Providing health and wellness education and outreach and community training; assisting with phased program design and development
-Ensuring effective care management that includes medically-related as well as social wellbeing aspects as detailed in case management plans and/or directed by providers.
-Conducting initial and ongoing community health and wellness assessments.
-Serving as the liaison between clinical staff and external programs and services including those provided by the Tribe, local government, state and/or federal entities.
MINIMUM QUALIFICATONS:
-Bachelor’s degree in public health or affiliated field and/or a Washington Advanced Registered Nurse Practitioner, Family Nurse Practitioner, Registered Nurse or Practical Nurse license.
-Minimum of two years of supervisory experience (required).
-Minimum of two years of successful program management (preferred).
-Experience in program and/or strategic planning (preferred)
-Experience coordinating or managing a clinical and/or public health program (preferred).
-Public speaking and presentation experience (preferred).
-Prior experience working with Native American Indian population (preferred).
